Clicking 'Next' on the registration form does absolutely nothing

Why is it impossible to get past page 1 of this PPMA form? All personal details are entered correctly, but the button to head to the next section isn’t doing anything at all. Even tried clearing the browser cache and restarting the application, but the glitch is still there. If anyone knows a workaround to get to page 2, please drop a comment.
